We run all our surgeries by an appointment only. Appointments can be booked up to eight weeks in advance or on the day if you wish to be seen urgently.
We try to make sure you see the doctor of your choice but if they are not available we may ask you to see another doctor.
Please telephone our appointments line on 01522 876800 and a member of the reception staff will discuss your requirements and endeavour to make an appointment with the most appropriate practitioner at a convenient time (wherever possible). Please consider using our text reminder service? simply email us on firstname.lastname@example.org, or tell us when you phone.
The Receptionist may ask you for an indication of the reason for the appointment request, this is to ensure that you are given an appointment with the most appropriate person, you do not have to give this information, it is purely to give you an appointment that will be of most benefit to you.
In many cases it’s worth considering an appointment with a practice nurse rather than a doctor. Practice nurses are qualified to deal with many conditions and you may be seen more quickly. See the above tab for more information.
We are a Training practice so GP Registrars will be working with our team from time to time. Relevant information will be displayed on our waiting room screen as appropriate.
This service is available even when we are closed, so you can use it at virtually any time of the day or night, including at weekends

Portland Medical Practice is pleased to be able to offer our patients access to routine appointments at evenings and weekends
The new service is part of an extended access service which has been commissioned by NHS Lincolnshire West Clinical Commissioning Group.
The appointments will also be available on bank holidays and across the Easter, Christmas and New Year periods
This new service commences on 3rd September 2018. Extended hours appointments are for pre-bookable non-urgent appointments. Examples of these include chronic illness, asthma checks and medicines reviews,
The service is being delivered by a federation of GP practices in the area. If a patient wishes to attend an extended hours appointment, they may need to travel to another local GP practice or a central GP practice hub and may not necessarily see their own GP.
In our locality, appointments are available between 6.30pm and 8pm Monday to Friday, and on Saturday and Sunday mornings. There are also a number of additional appointments being made available during core hours (8.00am to 6.30pm Monday to Friday).

Cancellations - If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else. If a patient repeatedly fails to attend for appointments they may be removed from the practice list and asked to find an alternative practice. To cancel your appointment please telephone 01522 876800, we have a new cancellation line to make this process faster for patients.
We have three senior nurses working at an advanced level as nurse prescribers. You may not always need to see a doctor for your problem and may be offered an appointment with a nurse prescriber as an alternative. They are able to deal with the following:-
Diarrhoea + Vomiting
Ingrown Toe Nail
UTI (Adult Females
Whilst we encourage our patients to come to the practice, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception before 10:00am. The Doctor will need your contact telephone number. Home visits will usually be made between 12:30 to 14:30.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls
You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Employees must give their employer a doctor’s ‘fit note’ (introduced April 2010 - formerly sick note) if they are off ill for more than 7 days. Hospital doctors or GPs provide them. They can charge a fee if a fit note is asked for before the 7th day.
The fit note will say the employee is either ‘not fit for work’ or ‘may be fit for work’.
If it says the employee ‘may be fit for work’, employers should discuss any changes that might help the employee return to work (e.g. different hours or tasks). The employee must be treated as ‘not fit for work’ if there is no agreement on these changes.
Employers can take a copy of the Fit Note, the employee should keep the original.
Employers can also ask employees to fill in a form when they return to work to confirm they’ve been off sick for up to 7 days. This is called ‘self-certification’. Employers usually provide their own version of this form.
